Summary:
Provide wraparound case management for clients in mobile outreach and treatment programs.
Responsibilities:
- Link clients to housing, employment, and medical services
- Maintain client progress and service documentation
- Collaborate with outreach and medical teams
Qualifications:
Experience with mobile or street outreach preferred
Bachelor’s in Human Services, Social Work, or similar
Valid DE driver’s license and reliable transportation
